REQUIREMENTS AND PROCESS

For First-Time Applicants:

1. Birth certificate from the{{1}} National Statistics Office (NSO) or a certified true copy of the birth certificate from the Local Civil Registry and duly authenticated by the NSO
2. Valid IDs with pictures and supporting documents
3. For married women: Marriage contract{{2}} issued by the NSO or a certified true copy of marriage contract issued by the Local Civil Registry and Duly authenticated by NSO
4. Accomplished application form

Requirements for Passport Renewal

Brown Passport or Passport Issued before March 1, 1995

• Original passport and photocopies of its pages 1-4 and the last page
• Birth certificate from the NSO or certified true copy of the birth certificate from the Local Civil Registry

Green Passport

• Original passport and photocopy of first and last page

Machine Readable Passport

• Original Passport and photocopy of first and last page
• If place of birth or other data is incomplete: birth certificate from the NSO or certified true copy of the birth certificate from the Local Civil Registry

Electronic Passport

• Original passport and photocopy of first and last page

Replacement of Lost Passport

• Submit same requirements as that of first-time applicants
• Notarized affidavit of loss
• Police report (if passport is still valid)

Process

1. Call the DFA hotline (02) 737-1000 and schedule an appointment, or log on to the DFA appointment website (http://passport.com.ph/set-appointment).<o:p></o:p>
2. Go to http://passport.com.ph/print to download and print an application form.
• If the appointment was scheduled through the DFA hotline, use the print code given by the customer care officer.
• If the appointment was scheduled through the DFA website, a reference number and link will be emailed to the applicant.
3. Go to the DFA office on your scheduled date and time of appointment.
4. Present the printed and duly accomplished application form at the Appointment Center.
5. Get your queue number and wait for your turn.
6. After processing, proceed to the Passport Enrolment Section and pay the processing fees.
7. Proceed to the Encoding Section to have all the necessary data captured.
8. Date of release will be given.
• Passports may be released by delivery without returning to the DFA office. This service is optional.
• If you are availing of this service, go to the Delivery Counter and pay the delivery fee.

Passport Processing Fees

Express Processing:

• 7 working days
• P1,200.00

Regular Processing

• 15 working days
• P950.00

Additional Fee: P 200 for a lost valid green/machine-readable passport and P 350 for lost valid e-passport.

REMINDERS

1. Birth certificates should be printed on Security Paper (SECPA) issued by the NSO or should be a certified true copy from the Local Civil Registry duly authenticated by NSO.
2. Have all your original documents, including IDs, photocopied before going to the DFA. Applicants will not be admitted if their documents are not photocopied; and
3. if they do not have valid appointment and application forms.
4. The DFA may require additional documents to support passport application, especially for applicants with dual citizenship.
5. Personal appearance is required. Strictly no escorts.

6. In case of a missing passport, a notarized affidavit of loss must be produced stating when, where and how the document was lost. If possible, prepare photocopies of lost passport.
7. Applicants will only be entertained on the exact time of their appointment.
8. During the photo-taking portion of data capturing, applicants are not allowed to wear earrings and contact lenses, or to expose their teeth while smiling.
9. Check all the data in the computer monitor and in the enrolment certificate before signing it.
10. The number of processing days may vary depending on the location of the regional consular office.
11. Please claim your passport on the scheduled date. After six months of their release, unclaimed passports will be cancelled.

E-9 foreign worker quota fixed

The government has fixed the 2114 E-9 visa quota for foreign workers at 53,000, an increase of 3,000 from this year, the Ministry of Employment and Labor said Sunday.

The figure comes as nearly 17,000 workers are expected to return to their home countries next year after their contracts expire and additional demand for foreign workers from small- and medium-sized firms is estimated at 36,000.

The government said the vast majority of foreign workers (some 42,000) will be employed in the manufacturing sector while the remainder will work in the agriculture and fisheries industries.

The maximum quota for H-2 visas for ethnic Koreans from China and the former Soviet Union region, widely known as the Commonwealth of Independent States, will remain at 303,000, the same as that of this year.

The government confirmed the quota during a policy committee meeting on foreign workers. It said firms providing dormitories for foreign workers will be given advantages in securing labor.

It plans to unveil a comprehensive plan to prevent industrial injuries among migrant workers in January.
